Samahan Joins South Bay Health Care Patnership

National City, Calif.---   In an effort to lessen the number of patients utilizing hospital emergency services within the South Bay region of San Diego,  the Access to Care Coordination Initiative (ACCI) was formed.  Funded by the San Diego County Department of Health and Human Services, ACCI   aims to identify and assist the following types of patients find a medical home. One, those who are seen at the hospital’s emergency room and then are required follow-up care; and two, those who show up at the hospital’s ER but do not require life-threatening care.
A medical home is a cost-effective healthcare setting that offers a highly personalized system of care for each patient. It is a place where patients can comfortably visit for their regular health exams.  

ACCI is a partnership between Scripps Memorial Hospital and the following community clinics that offer as potential medical homes: San Ysidro Health Center, La Maestra Clinic, Imperial Beach Health Center and Operation Samahan.   Patients are given a choice from among the participating ACCI community clinics.   Referrals from the network are tracked through a special software developed by Scripps Hospital. This allows for an efficient exchange of information between Scripps and the participating clinics and consequently, enables each participating clinic keep track of the patients’ overall health care status.
